Before your appointment
Referral
Needed for bulk-billed testing. Bring it, or ask your doctor to send it to us. GPs can use our referral forms.
Medicare card
Bring your Medicare card (and DVA card if you have one). See fees & Medicare.
Your equipment
For CPAP appointments, bring your machine, mask, tubing and power cord. For asthma education, bring all your inhalers and spacers.
